The transition from Acute Kidney Injury (AKI) to Chronic Kidney Disease (CKD) is often underrecognized, resulting in missed opportunities for intervention and follow-up care. This session explores the kidney disease continuum, including Acute Kidney Disease (AKD), and examines the factors that influence recovery versus progression. Participants will analyze clinical scenarios to strengthen decision-making related to post-AKI monitoring, risk identification, and patient education strategies aimed at reducing long-term complications.
